The following form is designed to gather basic information about all program participants, along with optional information that can be used for the purpose of matching mentors and mentees. Mandatory fields are marked with an asterisk (*). Our mission is to ensure that all mentees and mentors participating in this program have the opportunity to form meaningful and positive connections with one another, allowing for open exchange of ideas, support, and guidance. The goal of allowing participants to elect what information they share -- and subsequently assign a degree of importance to optional criteria -- is to facilitate, where possible, matches that are tailored to suit mentees’ and mentors’ preferences.
The full registration form will take ~5-30min to complete depending on how detailed you make your answers as well as which questions you choose to respond to. You are welcome to fill in as little or as much as you like, though bear in mind that the more information you provide, the better you will be able to be matched. Once you have completed the first page of the forms and created an account, you can leave and return to complete the rest of the form whenever you choose.
